Parent / Supervisor Agreement
We have reviewed the Student Application and to the best of our knowledge, all information provided is complete and accurate. We understand that we are responsible for the tuition payment, which provides for all necessary curriculum assignments, parent answer keys, report cards, and permanent record file. We understand that the registration fee is a one-time payment and is non-refundable. We have read and understand the refund policy of the Academy. Link to Refund Policy We understand that a school year is a maximum of 12 months, beginning on the date of enrollment and ending when the student has completed all curriculum assignments. If enrolling from another school during a school year, we understand that the school year start date is the start date of the previous school and curriculum assignment adjustments will be made by the Academy staff. We understand that if we choose the Quarterly Tuition Payment, it provides for (1/4) one quarter of the school year of materials and should be completed within (3) three months. We understand that a student is placed on the Inactive Roll if we do not send another tuition payment within the three month period and that a Reactivation Fee of $10.00 will be assessed to reactivate our child in the Academy. We further understand that if we do not reactivate our child within (3) three months of his/her Inactive Roll status, our child will not be allowed to return to the Active Roll of this Academy and we will need to seek other educational alternatives. We also understand that if our child is placed on the Inactive Roll more than (2) two times and accumulates more than (4) four months of Inactive Roll status in one school year, our child will not be allowed to return to the Active Roll of this Academy and we will need to seek other educational alternatives. We further understand that if completed assignments are returned after exceeding the Reactivation deadline, the Academy will assess an Extension Fee of $25. We understand that if we choose the Full-Year Tuition Payment, it provides for one school year of materials and should be completed within (12) twelve months. We understand that a student is placed on the Inactive Roll if we do not send another tuition payment within the twelve month period and that a Reactivation Fee of $10.00 will be assessed to reactivate our child in the Academy. We further understand that if we do not reactivate our child within (3) three months of his/her Inactive Roll status, our child will not be allowed to return to the Active Roll of this Academy and we will need to seek other educational alternatives. We further understand that if completed assignments are returned after exceeding the Reactivation deadline, the Academy will assess an Extension Fee of $25. We understand the importance of continual communication channels with the Academy and will contact the Academy office immediately with any changes in our physical, mailing, email addresses, or phone numbers. We further understand that if all communication channels are broken, the student will be withdrawn by the administration of the Academy. We understand that our child is to be under direct adult supervision during study time and that the supervising adult will read and adhere to the Academy policies and procedures as set forth in the Academy Handbook of Instructions. We agree that the supervising adult will submit our responses to the Academy's Required Compliance Forms that are sent to us via email. We further understand that the supervising adult will require the student to adhere to the Academy procedures, including, but not exclusive of, not copying answers from the Score Keys or Test Keys and by keeping the Test Keys in a secure location, or any Program Abuse (cheating). We agree to submit completed and graded Tests at the end of the first month of enrollment and at the end of each semester term (half-year). We understand that Christian Academy of America reserves the right to withdraw students and withhold grades of any student that is found to be in Program Abuse or parent in Non-Compliance of Academy procedures, and will be charged a Reactivation Fee of $10.00, if reinstated. By entering our names in the Signature boxes, we understand and agree with the above statements and also verify the information entered is correct to the best of our knowledge.

We have reviewed this New Student Application, and, to the best of our knowledge, all information provided is complete and accurate.
We understand that the school year begins on our student’s official Start Date, which is determined by CAA. The school year ends when our student has completed all assigned coursework, or we have reached the maximum time allowance of twelve months. If transferring from another school during a school year, we understand that the school year start date is the start date of the previous school, and curriculum assignments and time allowances will be adjusted accordingly by the Academy staff.
We understand that we are responsible for tuition, which covers all necessary standard curriculum and answer keys, report cards, accreditation, and permanent records. We understand that the registration fee is a one-time payment and is non-refundable. We have read and understand CAA’s Refund Policy for the workbook program.
We understand that tuition installments are not charged automatically unless we specifically request it. If we would like to set up automatic recurring payments, we need to contact the Academy staff. We understand that recurring payments are scheduled for one grade level at a time and must be requested again for each subsequent school year.
We understand that if we choose the Quarterly Tuition Option it covers one quarter (1/4) of the school year’s tuition and materials, and the quarter of work must be completed within three (3) months of the official start date or, for subsequent quarters, within three (3) months of the previous tuition due date. We understand that our student will be placed on the Inactive Student List if we do not submit another quarterly tuition installment within the three-month period and by the specified tuition due date, and a reactivation fee of $15.00 will be required to reactivate our student in the Academy.
We understand that if we choose the Semesterly Tuition Option it covers one half (1/2) of the school year’s tuition and materials, and the semester of work must be completed within six (6) months of the official start date or, for the second semester, within six (6) months of the previous tuition due date. We understand that our student will be placed on the Inactive Student List if we do not submit another semesterly tuition installment within the six-month period and by the specified tuition due date, and a reactivation fee of $15.00 will be required to reactivate our student in the Academy.
We understand that if we choose the Full Year Tuition Option, it covers one school year (grade level) of tuition and materials, and the school year of work must be completed within twelve (12) months of the official start date. We understand that our student will be placed on the Inactive Student List if we do not submit tuition for the new year within the twelve-month period and by the specified due date.
We understand that if we do not reactivate our student within three (3) months of his/her being placed on the Inactive Student List, our student will not be reactivated or allowed to continue the school year with CAA, and we will need to seek other educational alternatives. We also understand that if our student is placed on the Inactive Student List and accumulates more than three (3) months of inactive status in one school year, our student will not be reactivated or allowed to continue with CAA, and we will need to seek other educational alternatives.
We understand that the cost of shipping original materials to students is covered by tuition. We understand that we are responsible to compare the materials received with the packing slip, and if anything is missing, we must notify the Academy office within ten days of receiving the curriculum. We understand that claims made after ten days will incur replacement and shipping fees. Additionally, we understand that materials sent by CAA as a replacement for work that has been lost, damaged, or changed due to course adjustments will incur replacement and shipping fees.
We understand that return shipping of the various reports covered in the handbook, including all CAA-owned resources (i.e., score keys, test keys, and resource books), and any supplies needed to ship those items back to CAA are our responsibility. We understand that CAA highly recommends we use a shipping method with a tracking number and insurance. We further understand that if work is returned with postage due, we will be charged.
We understand the importance of reliable communication channels with the Academy and will contact the Academy office immediately with any changes in our mailing address, shipping address, email address, or phone number(s). We further understand that if CAA staff is unable to reach us using the contact information provided, our student may be withdrawn by the administration.
We understand that our student should be under direct adult supervision during study time and that the supervising adult must read and adhere to all Academy policies and procedures outlined in the CAA Workbook Handbook. We agree that the supervising adult will respond to CAA's Required Compliance forms #1 and #2, which will be sent to us via email. We further agree that the supervising adult and our student will adhere to CAA procedures, including but not limited to not copying answers from the score keys or test keys, and keeping the test keys in a secure location. We agree to submit completed and graded tests at the end of the first month of enrollment and at the end of each semester (half year).
We understand that CAA reserves the right to withdraw students and withhold grades of any student that is found to be in Program Abuse (including but not limited to cheating and plagiarism) or whose parent/supervisor is noncompliant with CAA procedures and policies. If a student is reactivated after being withdrawn by the administration for any reason, the $15 reactivation fee will apply.
We understand that CAA reserves the right to request workbooks, tests, or other coursework at any time in order to verify the integrity of the coursework that is being completed and the grades that are submitted. We understand that we must keep all workbooks, worksheets, and a copy of the Master Record Sheet until three months after the report card date.
We understand that although this program is self-paced, students must make consistent, reasonable amounts of progress. We understand that if our student is not putting forth visible effort to complete their schoolwork and return the required reports in a timely manner, CAA reserves the right to withdraw them or put them on probation. We understand that the administration will not allow a student who is not progressing in a timely manner to remain enrolled.
Students who can progress through the curriculum at an accelerated rate are permitted to do so. However, they must meet the legal guidelines for enrollment (usually 180 days). Students taking a full-time course load (5 courses or more) must also meet the following minimum time requirements: semester course - at least 3 months; full-year course - at least 5 months.
We understand that if our student chooses to accelerate their progress (e.g., 3-4 months - semester; 5-6 months - full year), we must turn in all coursework, including workbooks, with the Mid-Year and End-Year reports.
We understand that the Reenrollment Form and $35 reenrollment fee are required at the beginning of each grade level for enrolled students.

We understand that if we choose the Monthly Tuition Payment, our student will have access to the online assignments for one month at a time. If we choose the Quarterly Tuition Payment, it provides access for two months and after the final quarter tuition payment is made, our student will have access until the twelve-month period has expired or that grade level is complete. We understand that if a tuition payment is late that our student will be placed on the Inactive Roll and that a Reactivation Fee of $10.00 will be assessed to the tuition payment to reactivate our student in the Academy. We further understand that if we do not reactivate our student within (3) three months of his/her Inactive Roll status, our student will not be allowed to return to the Active Roll of this Academy and we will need to seek other educational alternatives. We also understand that if our child is placed on the Inactive Roll and accumulates more than (4) four months of Inactive Roll status in one school year, our student will not be allowed to return to the Active Roll of this Academy and we will need to seek other educational alternatives. We understand that if we choose the Full-Year Tuition Payment, it provides for one school year of online access and should be completed within (12) twelve months. We understand that a student is placed on the Inactive Roll if we do not send another tuition payment within the twelve-month period and that a Reactivation Fee of $10.00 will be assessed to reactivate our student in the Academy. We also understand that students are required to complete at least one project per unit, except for P-B Electives (Project-Based Electives). We understand that Christian Academy of America reserves the right to withdraw students and withhold grades of any student that is found to be in Program Abuse (cheating or plagiarism) or in Non-Compliance of Academy procedures, and will be charged a Reactivation Fee of $10.00, if reinstated. We understand that if we choose the Monthly Tuition Option, it provides one month of access to the online assignments, and nine installments will be due for a full grade level. The Quarterly Tuition Option provides access for two months, and four installments will be due for the grade level. The Semesterly Tuition Option provides access for four months, and two installments will be due for the grade level. After the first tuition installment is paid (monthly, quarterly, or semesterly), subsequent installments must be made by specified due dates for our student to retain access to the online coursework and remain on the Active Student List. After the final tuition installment is made, our student will have access until the grade level is complete, or until our student has reached the maximum time allowance of twelve months that began on the official start date. We understand that if we do not submit subsequent tuition installments by the specified due dates, our student will be placed on the Inactive Student List, and a reactivation fee of $15.00 will be required to reactivate our student’s enrollment in CAA. We understand that all tuition installments for the grade level are due before a report card will be processed and our student promoted to the next grade level, even if they finish the work at an accelerated rate.
We understand that if we choose the Full Year Tuition Option, it provides one year (12 months) of access to the online assignments for that grade level, and all assigned coursework for the grade level must be completed within this time. We understand that our student will be placed on the Inactive Student List if we do not submit tuition for the new year within the twelve-month period and by the specified due date.
We understand that if we do not reactivate our student within three (3) months of his/her being placed on the Inactive Student List, our student will not be reactivated or allowed to continue the school year with CAA, and we will need to seek other educational alternatives. We also understand that if our student is placed on the Inactive Student List and accumulates more than three (3) months of inactive status in one school year, our student may not be reactivated or allowed to continue with CAA, and we will need to seek other educational alternatives.

We understand that our student should be under direct adult supervision during study time and that the supervising adult must read and adhere to the Academy policies and procedures outlined in the CAA Online Handbook. We agree that the supervising adult will respond to CAA's Required Compliance forms #1 and #2, which will be sent to us via email. We further agree that the supervising adult will keep the Ignitia teacher ID and password in a secure location and that no one except the supervising adult will have access to it.
We understand that all required assignments must be completed before credit is awarded or a student is promoted to the next grade. No originally assigned lessons, quizzes, or tests may be skipped. We also understand that high school students are required to complete at least one project per unit in most courses. For project-based electives, all originally assigned projects must be completed.
